The coexistence of mental health and a substance use disorder is called dual diagnosis or co-occurring disorders. Either disorder (drug addiction, mental disorders) can develop first. Despite this, dual disorders can interact and make each other worse.

  • According to SAMHSA’s 2021 National Survey on Drug Use and Health, approximately 9.2 million adults in the United States have a co-occurring disorder. 

  • In Georgia, around 9.2% of adults experienced a major depressive episode, and 18.4% of adults reported any mental illness in 2020, as per SAMHSA.

  • Anxiety, depression, bipolar disorder, ADHD, and schizophrenia were the leading mental health disorders to co-occur with a substance use disorder, reports The National Institute of Mental Health (NIMH). Alcohol, cannabis/hallucinogens, opioids, and sedatives were the most commonly misused substances.  

  • The same report states that 6.7% of adults in Georgia had a substance use disorder in the past year.

Depression

Depression is a medical condition that affects the brain and is more than just feeling sad for a few days. It’s a serious illness that consumes your daily life. In the U. S., more than 20 million people have depression, and it doesn’t go away on its own. In severe cases, it can lead to suicidal ideation.

There is a strong link between depression and substance abuse. Many people with depression self-medicate with drink or drugs to boost their mood or escape feelings of guilt or misery. Alcohol is the most prevalent drug for depressed individuals, but this doesn’t have to include you.

  • Nearly one-third of people with major depression also have an alcohol problem. 

  • Reaching for drugs or alcohol to lift spirits has the opposite desired effect. Substances like alcohol cause symptoms of depression and substance abuse to worsen. It’s a vicious cycle. 

  • The Substance Abuse and Mental Health Services Administration in Georgia reports that approximately 7.6% of adults had a major depressive episode and a substance use disorder in the past year. 

  • Additionally, 5.5% of adolescents (ages 12-17) in Georgia had a major depressive episode in the past year, and 8.2% of adolescents had a substance use disorder in the past year. 

  • Furthermore, data from the National Survey on Drug Use and Health showed that in 2019, among adults who experienced a major depressive episode, 19.5% had a co-occurring substance use disorder.

Anxiety Disorders

Anxiety is a feeling of worry, nervousness, or fear about something that might happen in the future. The complex disorder can cause physical symptoms such as a racing heart or sweating.

Anxiety and substance abuse can look alike, making it tough to know if someone has anxiety or addiction problems. People with anxiety may turn to alcohol or pills as coping mechanisms.   • Studies show that 20% of individuals diagnosed with an anxiety disorder also have a substance use disorder. According to a study conducted in Georgia, PTSD (6.6%) and GAD (5.8%) were the most common anxiety disorders identified.

  • Between 18% and 28% of people in the United States have an anxiety disorder during any given year. Within this percentage of individuals, there is a 33% to 45% rate for a co-occurring substance use disorder.

  • According to  SAMHSA, in 2019, 7.2 million adults in the United States had co-occurring mental health and substance use disorders. Anxiety and depression ranked as the most common mental health disorders.

Schizophrenia

Schizophrenia is a serious mental health condition affecting a person’s thinking, behavior, and emotions. It is a psychotic disorder that can cause a person to lose touch with reality. The disorder can cause them to hear things that aren’t there, believe things that aren’t true, and have trouble doing everyday things. 

People with schizophrenia often turn to drugs and alcohol, making their symptoms worse and making it harder to treat their illnesses. Addiction and not sticking to a med routine never end well. Southeast Detox wants to help you reclaim your life.

  • The prevalence of dual diagnosis is high for individuals with schizophrenia. In fact, up to 50% of individuals with schizophrenia also have a co-occurring substance use disorder. 

  • People with the disorder have the highest prevalence of smoking (70 to 80%), with rates up to 5 times higher than the general population.

  • Another challenge in dual cases involving schizophrenia is the resistance to sticking to a medication routine. A study found that after one year, over 50% of the sample group had either stopped taking medication or were non-compliant.

  • Alcohol use disorder (AUD) is the most common co-occurring disorder in people with schizophrenia. Both biological factors and psychosocial factors are thought to contribute to this co-occurrence.

  • The illicit drugs used most by patients with schizophrenia are alcohol, cannabis, and cocaine. 

Bipolar Disorder

Bipolar Disorder is a two-faced mental illness that affects a person’s mood, causing extreme highs and ultra-lows. Substance use can also trigger or worsen mood episodes in people with bipolar disorder. In addition, there may be a genetic or environmental link between bipolar disorder and substance use disorders.

  • Individuals with bipolar disorder are more likely to also have a substance use disorder, with rates as high as 60%.

  • In Georgia, it is estimated that 4.6% of adults have experienced a bipolar episode at some point. Bipolar affects approximately 2.8% of the adult population in the United States.

  • 24.2% of those surveyed with bipolar disorder experienced a problem with cocaine abuse in their lives. Evidence suggests that periods of cocaine use may worsen the symptoms of bipolar disorder and contribute to the illness getting worse.

  • A study revealed that individuals with bipolar disorder since adolescence have a greater risk of a substance use disorder later compared to developing the disorder as adults.

Personality Disorders

Personality disorders are mental illnesses where a person’s thoughts, feelings, and actions aren’t considered normal. These mood difficulties are usually constant across situations, harming relationships and personal care.

Affected individuals may be more likely to fall into drug or alcohol addiction due to their unstable emotions, discomfort, or impulsiveness. Self-medication as a coping skill is a cycle that worsens mental health issues. Individuals reliant on drugs or alcohol are also more resistant to attending therapy.

Post-Traumatic Stress Disorder (PTSD)

Post-traumatic stress disorder occurs from witnessing or experiencing shocking and dangerous events. Triggers include physical violence, sexual assault, military combat, natural disasters, or serious accidents. 

PTSD is a chronic type of anxiety disorder. PTSD can cause intense feelings of fear, anxiety, and depression. PTSD can affect a person’s ability to function in their daily life. Individuals often grow distant from loved ones, develop multiple addictions, and use substances to numb their trauma.   • Trauma-targeting treatment options like prolonged exposure (PE) and cognitive behavioral therapy (CBT) have a large evidence base with a trauma focus. These therapies address memories and thoughts related to the event.
